Connections for Community is a Townsville-based nurse-led team, dedicated to providing personalised in-home nursing support and practical care navigation for individuals and families who are recovering at home, managing illness, or feeling unsure how to access services. We work in partnership with GPs and other care providers to ensure continuity of care, helping people feel heard and reducing overwhelm. We travel across regional and rural areas, ensuring communities outside major centres receive support.

We partner with general practices by providing home-based nursing visits for patients who cannot reliably attend a clinic. This includes housebound patients, individuals who have disengaged from regular care, and those moving through complex health transitions where continuity and monitoring are especially important. We extend our reach beyond the practice, aligning our treatment with your clinical plan.

Brokered Nursing

Right care, right place,
right time

Brokered nursing gives patients, families, and health professionals a simple way to organise the right nurse at the right time. For patients and families, it means getting trusted in-home nursing support without having to navigate the system alone. For GPs, it offers a reliable way to extend care outside the clinic. For community organisations, hospital teams, NDIS coordinators, and other health professionals, it ensures timely, coordinated nursing that fills service gaps and supports safe transitions.
